Request 1095-C Form

For Current and Former GLSS Employees:

Due to new federal legislation designed to reduce paperwork and administrative burdens, we are informing you of changes related to distribution of the 1095-C form.

The 1095-C form provides information about the health insurance coverage GLSS offered you throughout the year.  You are not required to file this statement with your personal taxes.

Federal law now requires that GLSS furnish a copy of the 1095-C form to an employee only upon request
